Summary:

Equal Hope is a non-profit sitting on the Rush University Medical Center Campus. Equal Hope is dedicated to eradicating women’s health racial, ethnic and class disparities and is a mission-oriented healthcare justice organization. Equal Hope is seeking a dynamic and experienced Navigation Manager to lead our patient navigation team and oversee navigation efforts, including site coordination and management of the Illinois Breast and Cervical Cancer Program (IBCCP). The Navigation Manager will play a crucial role in ensuring seamless navigation services for individuals accessing breast and cervical cancer screening and treatment services.

Required Job Qualifications:

• Registered Nurse (RN) license in the state of Illinois.

• Bachelor's degree in healthcare administration, public health, social work, or a related field.

• Experience in program management, patient navigation, or healthcare coordination.

• Knowledge of cancer screening programs, specifically breast and cervical cancer screening.

• Strong leadership and team management skills.

• Excellent communication and interpersonal skills.

• Data management and analysis capabilities.

• Ability to work collaboratively with diverse stakeholders.

• Commitment to health equity and advocating for underserved populations.

• Proficiency in Microsoft Office suite and electronic health record systems

Preferred Job Qualifications:

• Bachelor of Science in Nursing (BSN) or Master’s preferred.

Responsibilities:

• Lead and manage the patient navigation team, providing guidance, support, and mentorship to staff.

• Coordinate navigation services across multiple sites, ensuring efficient and effective delivery of care.

• Oversee the implementation and management of the Equal Hope navigation programs including the Illinois Breast and Cervical Cancer Program, ensuring compliance with program requirements and objectives.

• Monitor and evaluate the quality of navigation services provided, implementing improvements as needed.

• Establish and maintain partnerships with healthcare providers, community organizations, and stakeholders to enhance navigation services.

• Ensure accurate and timely documentation of patient interactions and program outcomes.

• Develop and implement training programs for navigation team members to enhance their skills and knowledge.

• Conduct performance evaluations of navigation team members and provide constructive feedback for professional growth.

• Manage resources efficiently to meet program goals and objectives within budgetary constraints.

• Advocate for patients' needs and rights within the healthcare system.

• Ensure adherence to all relevant policies, regulations, and accreditation standards.

• Identify areas for improvement in navigation services and implement best practices.

• Partake in strategic planning processes to enhance navigation services and program impact.

• Engage with the community to raise awareness of navigation services and increase program reach.

• Prepare regular reports on program performance, outcomes, and challenges for internal and external stakeholders.

• Explore opportunities for expanding navigation services and reaching more individuals in need of cancer screening and treatment support.
