A New Position in Care Coordination?
The RN Care Coordinator for the Rush Health Care Management Program is an Illinois licensed Registered Nurse who provides patient care through a diverse role as a clinician, educator, care coordinator, advocate, and quality improvement facilitator.
Independently works in a collaborative relationship with other members of the healthcare team to improvedelivery of comprehensive, cost-effective patient and family-centered care.
Offers expert advice in prevention, health maintenance, chronic disease management, and clinical evaluation of medical and psychosocial needs.
Responsible for all activities related to planning, development, management, oversight, and continued growth of the Rush Health Programs.
Position will be located at Rush Oak Park Hospital.
Flexible work hours: Monday – Friday, No Weekends, No Holidays!
Works with County Care medicaid adult and pediatric population to establish and maintain individualized plans of care to meet patients’ needs.
Effectively establishes, builds, and maintains rapport with patients and family members.
Initiates interventions based on physician-approved protocols and standing orders (i.e. referrals to ophthalmology and age-appropriate preventive screenings such as mammograms, colonoscopy, smoking cessation counseling, etc.).
Refers patient/family to self-management support programs as needed.
Follow up with patients discharged from the hospital or Emergency Department to ensure rendering of aftercare supports and services.
Responsible for quality improvement initiatives including data collection, chart review, interdisciplinary collaboration, and analysis of patient data.
Maintains knowledge and understanding of all current evidence-based guidelines and objectives for patient care coordination/disease management.
Position Qualifications Include :
Bachelor’s degree in Nursing required.
Current state of Illinois RN licensure required.
A minimum of two years of ambulatory nursing experience preferred, preferably with a focus in utilization management or case management.
Current CPR certification required.
Bilingual English/Spanish strongly preferred.
Strong written and verbal communication skills are required.
Strong critical thinking and analytical skills are required.
Proficient with Microsoft Office applications including Excel, Outlook, Word, Access, and PowerPoint.
Ability to function independently and prioritize effectively.
