If so, the RN Care Manager for patients receiving treatment in our Rush Cancer Center may be the opportunity you have been looking for.
Utilizes the CMS Oncology Care Model (OCM) which is an innovative, multi-payer model focused on providing higher quality, more coordinated oncology care.
Participates in the new care model, be an integral part of a multidisciplinary care management team composed of a nurse, social workers, a financial counselor and a patient care navigator.
Provides care coordination to support Medicare beneficiaries receiving treatment for cancer to ensure the delivery of high quality, efficient, timely, and cost-effective health care services.
Supports and provides outreach to patients who have recently been discharged from the hospital or ED.
Assesses the health care, educational, and psychosocial needs of the patient/familyand uses standardized tools to identify patients that are at risk for poor health outcomes.
Works with care management team, patient's treatment team in the cancer center, all other providers of the patients care and patient and their caregiver to develop individualized care plan/goals.
Works to improve identified outcomes, including reducing ED visits, inpatient admissions, and promoting overall health and well-being among patients in the model.
Identifies extrinsic barriers to adherence (i.e. transportation, financial concerns or pharmacy coverage) and work with patient, care team and other available resources to alleviate barriers.
Facilitates communication with patients and the care team, coordinates referrals, and promotes optimal allocation of available resources.
Position Qualifications Include:
Current RN Licensure in the State of Illinois.
Bachelor's Degree (or higher) in Nursing required.
3 years of progressive nursing experience in hospital, ambulatory, or home health setting is required.
Prior experience in Oncology is preferred.
Prior experience in case management, care coordination, transition of care, and patient advocacy is preferred.
Prior experience working with patients who may encounter obstacles to care.
